Group 1: Personal Assistance & Home Care Services

We support individuals with daily living activities to promote independence and stability at home and in the community.

We support individuals transitioning into more independent living environments. Services may include Transition planning assistance, Home setup coordination, Basic household establishment support, Stability-focused integration

Non-Medical Transportation

We provide reliable transportation support to help individuals safely attend appointments, community programs, employment opportunities, and essential errands.

Transportation support helps participants maintain independence while staying connected to the services and activities that matter most.

Home Adaptations

We assist with home modifications that improve safety, accessibility, and daily living independence.

Home adaptations may include adjustments that make living spaces safer and easier to navigate, allowing individuals to remain comfortable and secure in their homes.

Home Delivered Meals

We ensure individuals have consistent access to nutritious meals delivered directly to their homes.

This service supports individuals who may have difficulty preparing meals themselves while promoting health, stability, and daily well-being.

Pest Eradication Services

We help maintain safe and healthy living environments by addressing pest-related issues that may affect a participant's home.

A clean and safe home environment is essential for comfort, health, and long-term stability.

Group 2: ODP Intellectual Disability & Autism Services

Serving Pennsylvanians with intellectual disabilities and autism through the Office of Developmental Programs (ODP)

Through Pennsylvania's Office of Developmental Programs (ODP), we support individuals with intellectual disabilities and autism to achieve greater independence, choice, and opportunity — living Everyday Lives in their communities. These services are funded through home and community-based services (HCBS) Medicaid waivers administered in partnership with county and Administrative Entities.

Home & Community Habilitation

We support individuals with intellectual disabilities and autism in accessing and using community resources, developing financial responsibility, and participating fully in community life.

Services can be provided by a trained agency staff member or by someone the individual and family already know who is qualified and willing to provide support.

Community Integration

We help individuals with intellectual disabilities and autism actively participate in their communities through structured, goal-oriented engagement. Building community presence develops confidence, connection, and a sense of belonging

Community Transition Services

We support individuals transitioning into more independent living environments

Our goal is long-term independence and successful community inclusion.

Supports Coordination

All individuals enrolled in Pennsylvania's Intellectual Disability (ID) service system receive Supports Coordination services. Our Supports Coordinators assist individuals in Developing their Individual Support Plans (ISPs), Choosing providers and informal supports, Locating, coordinating, and monitoring services and Ensuring services align with personal goals and preferences

Supported Employment — Job Finding Services

We help individuals with intellectual disabilities and autism prepare for and obtain competitive jobs of their choice.

Services include:
Interview skills training
Resume preparation
SSI and SSDI benefits counseling
Job searching and placement support
Career exploration and assessment

Non-Medical Transportation

We provide reliable transportation support to help individuals with intellectual disabilities and autism safely attend appointments, community programs, employment opportunities, and essential activities.

Supported Employment — Job Support Services

We provide ongoing periodic follow-up with individuals and their employers to support long-term job retention through:

Continued workplace training
Employer communication and support
Skill development on the job
Problem solving and conflict resolution

Workforce-Focused Community Integration

We support individuals in building the skills and connections needed to participate meaningfully in the workforce and community, bridging the gap between community inclusion and employment readiness
