What is Community HealthChoices?
Pennsylvania’s Medicaid program that helps eligible adults receive long-term care at home — instead of in a nursing facility.
Community HealthChoices (CHC) is a managed care program from the Pennsylvania Department of Human Services. It coordinates Medicare, Medicaid, and long-term services for adults 21 and older who need help with daily living.
Through CHC, qualifying Pennsylvanians can receive in-home care services — like bathing assistance, meal preparation, medication reminders, and companionship — often at no cost to the family.

ELIGIBILITY
Who Qualifies for CHC?
You may be eligible for Community HealthChoices if you meet the following criteria
Pennsylvania Resident
You must live in Pennsylvania to enroll in CHC.
Age 21 or Older
CHC serves adults 21 and above who need long-term care services
Medicaid-Eligible
Must qualify financially for Medicaid (Medical Assistance) coverage
Need for Care
Require help with daily activities like bathing, dressing, mobility, or medication.
Nursing-Facility Eligible
Determined by the state to need care typically provided in a nursing facility.
Dual Eligibility Welcome
Both Medicare and Medicaid recipients can participate in CHC.
